Lifescripts

Lifescripts is an initiative to assist general practice in patient risk factor management. The initiative includes Assessment and Prescription Tools for GPs to use when providing lifestyle and risk factor management for the following 5 risk factors;

  1. Smoking Physical Activity
  2. Nutrition Weight Management
  3. Alcohol

Sutherland Division of General Practice is integrating the Lifescripts initiative through all CDM program activities such as education, training and resource development. For example; the initiative has been incorporated into the education and resource development for the preparation and review of Medicare CDM Items, 45 Year Old Health Check and GP Exercise Referral Scheme. The following Lifescripts templates have been developed by the Australian General Practice Network with funding provided by the Australian Governmen Department of Health and Ageing.
