45 Year Old Health CheckAs part of the 'Australian Better Health Initiative', Medicare Australia has developed a service item for GPs to assist in the prevention of chronic disease. The Health Check is available to people aged between 45 – 49 years (inclusive) at risk of developing a chronic disease, with at least one risk factor. As with the Medicare CDM Items, the Division provides education and training to GPs, Practice Nurses and Practice Staff in the implementation of the 45 Year Old Health Check. The Division has integrated the Lifescripts initiative into this Health Check for the Assessment and Prescription of life style risk factor management. Item Information
