Southcare Services Directory - Sutherland Hospital and Community Health Services (revised 28 January 2011)
Southcare is a “one stop shop” for provision of quality services and care to older people and people with a disability who are residents of the Sutherland Shire.
- It offers a multiservice outlet whose mission is to enhance independence, quality of life and to optimise health and wellbeing. Southcare comprises Aged Care Assessment, Community Rehabilitation, Community Nursing and Home and Community Care funded services which together provide a comprehensive range of health services and social supports in collaboration with community agencies. It is responsible for determining eligibility for residential aged care and community aged care packages, support and rehabilitation for people at home, preventative and educational programs, carer support groups, day respite groups and services for people living with dementia. These services are carried out predominantly in the person’s own home in support of their own efforts and those of their families or carers.
- Uniquely, the service is also integrated with both acute hospital care and long term care, so as to assure continuity of care so often needed by the people we are assisting. Referrals to Southcare can be made by a health professional, a carer or the person themselves.
