Lifestyle Modification Programs and Preventative Health Checks

The Sutherland Division of General Practice (Shire GPs) Lifestyle Modification Program is a Government accredited and subsidised program which is designed to help people to prevent the onset of Type 2 Diabetes. The program is designed to help people improve their health. It provides access to education and support for various lifestyle risk factors including smoking, nutrition, physical inactivity and alcohol consumption. It allows individuals to adopt healthy lifestyle changes and behaviours.

Shire GPs' Lifestyle Modification Program incorporates a GP exercise referral scheme in partnership with Sutherland Leisure Centre where participants undertake a series of group exercise sessions. The GP Exercise Referral Scheme provides the physical activity component of the Lifestyle Modification program with other education sessions including nutrition, weight management, goal setting and pre-diabetes patient education.
