Chronic Disease Prevention and Management
Aims
The aims of the Chronic Disease Prevention and Management (CDM) Program are;
- To provide education and support to GPs and Practices for the management of patients with chronic diseases, in particular Type 2 Diabetes
- To provide GPs and Practices with the skills and resources for the prevention of chronic diseases through identification and management of lifestyle risk factors
Strategies

Lifestyle Modification Programs and Preventative Health Checks
Medicare CDM Items
Diabetes PIP/SIP - Recall & Reminder Systems
45 Year Old Health Check and other initiatives
Diabetes Data Aggregation Strategy
Aboriginal and Torres Strait Islander Australians
