Chronic Disease Prevention and Management

Aims

The aims of the Chronic Disease Prevention and Management (CDM) Program are;

  • To provide education and support to GPs and Practices for the management of patients with chronic diseases, in particular Type 2 Diabetes
  • To provide GPs and Practices with the skills and resources for the prevention of chronic diseases through identification and management of lifestyle risk factors

Strategies
