45 Year Old Health Check
As part of the 'Australian Better Health Initiative', Medicare Australia has developed a service item for GPs to assist in the prevention of chronic disease.
The Health Check is available to people aged between 45 – 49 years (inclusive) at risk of developing a chronic disease, with at least one risk factor.
As with the Medicare CDM Items, the Division provides education and training to GPs, Practice Nurses and Practice Staff in the implementation of the 45 Year Old Health Check. The Division has integrated the Lifescripts initiative into this Health Check for the Assessment and Prescription of life style risk factor management.
Item Information
- GP service item to assist with the prevention of chronic disease and to enable early intervention strategies
- Includes the GP (with assistance from Practice Nurse) conducting an overall assessment, patient history, examinations and investigations as required
- Development of interventions as indicated and providing advice to the patient
- Can only be claimed once for each eligible patient
- Rebate $100
Who is eligible?
- Patients aged between 45 and 49 inclusive
- Patient must have at least 1 risk factor for chronic disease
- Should be undertaken by patient’s usual GP
Division support to local GPs and practices
The Division provides education and training to GPs, Practice Nurses and Practice Staff in the implementation of the 45 Year Old Health Check. Training is provided through practice visits, workshops and CPD events. Contact Shona Dutton, Program Officer at the Division for further details.
